Before you call, make sure you have:
• The name of your referring physician (or primary care
physician)
• What your symptoms are
• What diagnostic tests you already have had and where
you had them done

If you already have an appointment you
can save time by completing the health history questionnaire
from home and only come to our office 10 minutes ahead
of your scheduled appointment time vs. the normal 45 minutes

This clinical intake questionnaire helps the neurosurgeon assess what is clinically going on with you, what makes your problem better or worse, and what have you have already tried before seeing us.
If you do not have a media program called Shockwave, you must first download and install a free Macromedia program called Shockwave. Follow the download instructions. Once Shockwave is installed, you will exit Macromedia and go back to the appointment page of our web site and click on the link under "If you already have an appointment". The Shockwave program will run and it will say Maryland Brain & Spine Neuro Consult, powered by Health Care Online. Then it should automatically go to the next screen in a few seconds and you should select "your first time seeing
us in the office" and answer the clinical intake questionnaire questions. Keep on selecting the next button to go to the next series of questions. You will know you have finished the questionnaire when it thanks you.
Prior to Your Visit To Our Office
To make your
appointment go smoothly, we ask that you come prepared
with the following items. Make sure you print out
and complete the forms for items numbers 5, 6,& 7)
prior to coming to our office.
1. All current radiological films and written radiologist
reports of MRI’s, CAT Scans, and X-rays. In order
to accurately assess your condition, the neurosurgeon needs
a
radiology exam & report that is less than six months
old or since your date of injury. Please ask your primary
physician
for a referral if you need one for a radiology study.
2. Your insurance card and picture id (license) so we can
easily identify you and determine who is financially responsible
for the visit.
3. A referral form if your insurance company requires one.
If your visit is related to a work or auto accident, you
must provide us with the name of the work or auto insurance
carrier, address to send claim to, claim #, date of injury,
name and telephone # of the claim adjuster, and the name & number
of attorney if you have one. You will also be asked to sign
a lien that protects our financial rights related to providing
care.
4. Any relevant medical
records from previous physicians/therapists
that would help our physicians understand your history.
5. A completed registration
form. Make sure to list the name, phone number and
address of your referring/primary care physician so we
can coordinate
your care with them. We will try to call you in advance of
your appointment to get you pre-registered in our computer
system to save you time.
6. A completed list of any medications you are currently
taking (including over the counter vitamins & supplements)
and allergies you have.
7. A completed Authorization
For Assignment of Benefits form.
8. Many patients find it helpful to write their questions
out ahead of time so they can be comfortable that the physician
covered every question.
9. New patients are asked to come in
45 minutes prior to your appointment so you can complete our
health history questionnaire (Neuro Consult) on our easy to
use touch screen that our physicians use to assess your condition.
Or you can access the same questionnaire by using home computer
to submit your information over our secure server by (clicking
here) then you only need to check-in 15 minutes ahead
of time.
We will try to call you and register you in
advance of your first visit so the check-in process goes smoothly.
What Happens During Your Office Visit:
1. If a determination is made that surgery is the most effective
option, you will be explained the risks and benefits of the
recommended procedure. We will provide you with written information
to take home and read so you can review everything discussed
during the consult before you make the final decision. For

of our web site.
2. If surgery is not an option, we will provide you and your
referring physician with some non-surgical recommendations.
We work with a network of physicians and therapists who get
excellent results using traditional and complementary therapies.
After Your Office VIsit:
1. If you desire to continue with the surgery, call our surgical
scheduling office to schedule your surgery. We can even perform
your pre-op exam.
2. In all cases, you and your referring physician will be
provided with a written report of our recommendations.
3. As an added convenience, we may also suggest and provide you with a neck or back brace prior to surgery so you can be comfortable using the corrective device.
After Your Surgery:
1. We will call you a couple days after surgery to make sure
you are recovering well.
2. You will have a series of post operative office visits
with members of the surgical team to check your progress.
3. After six and twelve months, we would appreciate your
logging back into Neuro Consult [click
here], and completing the post-surgical outcome questions.
Your answers will be used to improve our surgical procedures.
